How to Order

We have made the ordering process simple! 

Once you have successfully completed Step 1 and Step 2, you will receive

1. An email link that you can use to order Rapid Screener. 

2. A user account and access code so you can order Rapid Screener from any computer.

3. You may also choose to fax or phone in your request.

Necessary Information

Order Rapid Screener by completing our online referral form below  or by printing out an Adobe Acrobat form and faxing it to us. 

To complete the referral, you will need the following information:

1. Your Qualified User account number and access code

2. The names of the raters you wish to complete Rapid Screener

3. The raters' email addresses, fax numbers, or telephone numbers

4. The raters' preferred form of contact.

We will contact them on your behalf (it is generally is best if you let them know to expect that we will be contacting them).  We prefer email, but will contact them by whatever means is most convenient for them as specified by you.
